At Murdoch, your health, safety, and wellbeing are our top priorities. That’s why we’ve launched M-REPORT (Murdoch – Reporting Events, Prevention, Oversight and Risk Tracking) — our new hazard and incident reporting system.
M-REPORT is now live and available to all staff, students, and visitors, helping us all play a part in creating a safe and supportive community.
Who can use M-REPORT?
Students, staff and visitors to our campus are able to report hazards and incidents in M-REPORT. Employees and students will be able to log into the system with their Murdoch University credentials and QR codes will be available on campus for all to use, including visitors.
What should I report?
- Hazards: Anything that could cause injury, illness or harm to people, property or the environment.
- Incidents: Unplanned event or occurrence that has the potential to cause injury or illness.
- Near misses: Unsafe situations where no one was hurt, but it could have happened.
- Sensitive reports: For issues like psychosocial hazards, your report goes straight to the Health, Safety & Wellbeing Team to ensure confidentiality and support.
How do a I report a hazard or incident in M-REPORT?
Access M-REPORT on your computer or on your mobile phone. The link is also available on the Murdoch Safe App, and QR codes are displayed around campus.
To help you get started, Quick Reference Guides (PDFs) are available to step you through how to use M-REPORT:
